Following are links to some of the most frequently used University forms. These are pdf documents.

All Journalism and Mass Communications students will need to return the printed forms
to the Student Services Office, room 4001 Carolina Coliseum.

Change of College/Campus
To change major you must submit this form to the Student Services Office once you have been notified by your new major that you meet their entrance requirement. Download the form and type directly on the form.

Internship Contract
If you have obtained an internship for credit you will need to complete this form with our internship coordinator. Download the form and type directly on the form.

Special Enrollment Request
You must complete this form if you plan to study away from USC during a regular semester or during summer school.

Transcript Request
To request the University send your official transcript, this form must be returned to the Registrar's Office along with the nominal fee.

Transient Enrollment
If you plan to take a courses away from USC-Columbia during summer or any semester you must fill out this Transient Study Form, print, and submit the form with a copy of the course description(s) to the Student Services Office.

Undergrad Independent Study Contract
If you are working with a faculty member on a special academically related project and earning credit. Download the form and type directly on the form.
