Following are links to some of the most frequently used forms by students in the School of Journalism and Mass Communications. Many are pdf documents, which require Acrobat Reader.

All Journalism and Mass Communications students will need to return the printed forms to the Student Services Office, room 4001 Carolina Coliseum.

Academic Waiver
If you are requesting that the school waive an academic requirement, i.e.. allowed to take 500+ level JOUR courses without being in upper division or take an advanced JOUR course without the pre-req. Download application>

Senior Semester Application
